Download a "Claim Against the City of Santa Ana" form or call (714) 647-5470 to request that a claim form be mailed to you. If you are filing a claim after six (6) months after the date of incident, but not exceeding one (1) year, you also need to download an “Application for Leave to Present a Late Claim to the City of Santa Ana.” You may also pick up a claim form from either of the offices listed below:
